Insights
Management of unconjugated hyperbilirubinemia in preterm infants varies across Turkey. Prophylactic phototherapy for very low birth weight infants may improve care standards and prevent kernicterus.
Area of Science:
- Neonatology
- Pediatric Gastroenterology
- Public Health
Background:
- Prematurity is a major risk factor for unconjugated hyperbilirubinemia in newborns.
- Neonatal jaundice management requires standardized protocols, especially for preterm infants.
Purpose of the Study:
- To investigate current jaundice management practices for preterm infants in Turkey.
- To identify variations in hyperbilirubinemia treatment protocols among Turkish neonatal intensive care units.
Main Methods:
- A questionnaire-based survey was distributed to 100 level III neonatal intensive care units (NICUs) across Turkey.
- Data on jaundice management, including phototherapy guidelines and bilirubin monitoring, were collected from 84 participating centers.
Main Results:
- Significant variability exists in hyperbilirubinemia management protocols among Turkish NICUs.
- While 75.3% of centers follow Turkish Neonatology Society guidelines, 24.7% use alternative approaches.
- A majority (53.6%) believe prophylactic phototherapy is essential for infants under 1000g, and 6 cases of kernicterus were reported.
Conclusions:
- There is a lack of a unified standard for managing neonatal hyperbilirubinemia in preterm infants in Turkey.
- Incorporating prophylactic phototherapy for extremely low birth-weight infants into national guidelines is recommended to enhance care consistency and outcomes.
Background/Aim:
Prematurity is a significant risk factor for developing unconjugated hyperbilirubinemia. This study investigated the current approach to managing hyperbilirubinemia in preterm newborns in Turkey.
Materials And Methods:
A study-specific questionnaire on the management of jaundice in preterm infants was sent to 100 level III neonatal intensive care units in Turkey.
Results:
Responses were obtained from 84 centers from all regions of Turkey. Of the centers, 75.3% used the Turkish Neonatology Society guidelines for deciding to start phototherapy, and 24.7% used different guidelines. The monitoring of bilirubin varied among the participants. Of the participants, 53.6% believed that prophylactic phototherapy was necessary if the infant's birth weight was below 1000 g. The participants reported 6 cases of kernicterus in preterm infants in recent years.
Conclusion:
There is no single standard approach for managing neonatal hyperbilirubinemia in preterm infants in Turkey. Prophylactic phototherapy for extremely low birth-weight infants might be added to the guidelines for Turkey.
