Summary

Body mass index (BMI) does not accurately reflect body composition in women with metabolic syndrome (SM). Waist circumference and waist-to-height ratio show better cardiometabolic risk classification compared to BMI.

Background:

  • Body mass index (BMI) is widely used for nutritional status assessment in metabolic syndrome (SM).
  • However, BMI's ability to identify body composition differences in SM is not well understood.
  • Accurate assessment is crucial for managing cardiometabolic risk.

Purpose of the Study:

  • To determine if nutritional status variations reflect body composition changes in women with SM.
  • To identify the agreement between nutritional status and anthropometric indices for classifying cardiometabolic risk.

Main Methods:

  • Study included 136 women with SM (mean age 42 ± 3.5 years).
  • Evaluated nutritional status, muscle mass, fat mass, waist circumference (WC), waist-hip ratio (WHR), and waist-height ratio (WHtR).
  • Compared body composition and anthropometric indices; assessed cardiometabolic risk classification agreement with BMI.

Main Results:

  • Waist circumference (>88 cm) and WHtR (≥0.5) showed the highest agreement (94.9% and 94.1%) for classifying normal weight vs. overweight using BMI.
  • WC (>88 cm) better distinguished normal weight/overweight from obesity (85.3%).
  • WHR (≥0.85) demonstrated more homogeneous sensitivity and specificity for risk classification.

Conclusions:

  • BMI fails to identify body fat variations in women with SM based on nutritional status.
  • BMI shows better sensitivity than specificity compared to other indices for determining cardiometabolic risk in women with SM.
