Bioelectrical impedance analysis can predict worsening heart failure (HF) functional class. Changes in body composition, like decreased resistance/height, indicate a higher risk of deterioration over six months in HF patients.

Background:

  • Heart failure (HF) patients experience significant body composition changes.
  • Prospective studies linking these changes to functional decline are limited.

Purpose of the Study:

  • To determine if bioimpedance parameter changes predict New York Heart Association (NYHA) functional class deterioration over six months.
  • To assess the utility of body composition analysis in managing HF patients.

Main Methods:

  • 275 stable HF patients underwent bioelectrical impedance analysis (BIA) for body composition.
  • NYHA functional class was assessed at baseline and after six months.
  • Echocardiography confirmed HF diagnosis.

Main Results:

  • 24% of patients showed functional class deterioration; 76% improved or remained stable.
  • Higher extracellular water (>5%) correlated with hypervolemia.
  • A 5% decrease in resistance/height predicted functional class deterioration (OR 1.42, p=0.04).

Conclusions:

  • Bioelectrical impedance analysis is a valuable tool for identifying functional class deterioration risk in stable HF patients.
  • Body composition monitoring via BIA aids in predicting adverse outcomes.
