    • To assess the experience and resources of pathology departments in Maryland nonfederal hospitals concerning autopsies of dementia patients.
    • To identify challenges in performing and evaluating dementia patient postmortem examinations.

    • A survey was distributed to Chiefs of Pathology at all nonfederal hospitals in Maryland.
    • The survey collected data on hospital experience, availability of specialized techniques, and referral systems for dementia patient autopsies.

    • A majority of surveyed pathology services reported limited ongoing experience with dementia patient autopsies.
    • Many hospitals indicated a lack of specialized histochemical techniques or established referral systems for these examinations.
    • Difficulties in obtaining postmortem examinations were noted, particularly for patients dying outside of facilities with dedicated Autopsy Services.

    Conclusions:

    • There is a widespread deficit in expertise and resources for conducting comprehensive autopsies on dementia patients in Maryland hospitals.
    • Addressing these gaps is essential to improve diagnostic accuracy and advance research in dementia.
