    Background:

    • Multiple primary lung cancer is a rare nosological entity.
    • The incidence of multiple lung cancers is increasing due to earlier diagnosis and improved survival rates.
    • Smoking remains a significant risk factor for developing multiple lung cancers.

    Observation:

    • A 62-year-old patient presented with synchronous non-small cell lung cancer in the middle lobe and lower lobe of the right lung.
    • The patient had a history of invasive ductal adenocarcinoma of the right breast, treated with mastectomy and axillary dissection.
    • Diagnosis of synchronous lung cancer was made during a screening investigation.

    Findings:

    • Multiple primary lung cancer can be classified as synchronous (occurring simultaneously) or metachronous (occurring sequentially).
    • Accurate diagnosis is challenging, requiring exclusion of metastatic disease.
    • Meticulous staging, including MRI of the brain and FDG-PET scans, is essential for curative resection.

    Implications:

    • This case highlights the diagnostic and therapeutic challenges associated with multiple primary lung cancers.
    • Increased patient survival necessitates careful long-term monitoring for secondary malignancies.
    • Adherence to staging guidelines is critical for optimizing treatment strategies and patient outcomes.
