Summary

Sarcoidosis-related optic neuropathy, more common in women and certain ethnic groups, presents as inflammatory or progressive forms. Inflammatory types respond to corticosteroids but may relapse, while infiltrative types show less improvement.

  • To characterize the clinical presentation, subtypes, and treatment outcomes of optic neuropathy in patients with sarcoidosis.

Main Methods:

  • Prospective observational cohort study of 52 patients with sarcoidosis-related optic neuropathy.
  • Clinical assessment, MRI, and treatment response evaluation.

Main Results:

  • Optic neuropathy affected women more, with varied age distribution and higher prevalence in African/Caribbean groups.
  • Two subtypes identified: subacute inflammatory (resembling optic neuritis) and slowly progressive.
  • Corticosteroid treatment effective for inflammatory types; less so for infiltrative/mass lesions.
  • 25% relapsed, but relapse did not indicate a poor prognosis.

Conclusions:

  • Sarcoidosis-related optic neuropathy has distinct inflammatory and infiltrative subtypes.
  • Inflammatory optic neuropathy responds to treatment but is prone to relapse.
  • Infiltrative/progressive forms have a less favorable outcome despite treatment of the underlying sarcoidosis.
