  • To establish vital sign deviation thresholds for neonatal anesthesia and surgery.

  • To guide future research on reducing detrimental neurocognitive outcomes in neonates.

Main Methods:

  • Retrospective analysis of vital sign data in neonates (≤28 days) undergoing non-cardiac surgery (2010-2013).
  • Proposed thresholds for bradycardia, tachycardia, hypothermia, hyperthermia, hypertension, hypotension, hypocarbia, hypoxemia, and blood pressure variability.
  • Included chart review for risk factors and data artifact identification in 224 cases.

Main Results:

  • Analyzed data from 435 neonatal procedures; median age 6 days.
  • Observed an average of 5 vital sign deviations per case; only 11 cases had no deviations.
  • High incidence of hypothermia (70%), hypocapnia (69%), and severe hypotension (62%) noted.

Conclusions:

  • Developed an objective method to identify neonatal vital sign deviations during anesthesia.
  • The method has potential as a tool for quality improvement in neonatal anesthesia.
  • Further validation is needed to fully realize its potential in reducing adverse neurocognitive outcomes.
