Summary

Acupuncture may improve daily living activities and neurological function in stroke survivors, but evidence quality is low. More high-quality trials are needed to confirm these benefits and assess safety for routine use.

  • Searched multiple databases including Cochrane, MEDLINE, EMBASE, and Chinese medical databases up to July 2015.
  • Included trials compared acupuncture with sham acupuncture, placebo, or no acupuncture in ischemic or hemorrhagic stroke patients.
  • Main Results:

    • 31 trials with 2257 participants were included; most had low methodological quality and low to very low GRADE evidence.
    • Acupuncture plus standard treatment showed potential benefits in improving activities of daily living (Barthel Index), global neurological deficit, motor function, cognition, and reducing depression.
    • No significant differences were found when comparing real acupuncture to sham acupuncture for motor function and quality of life. Few adverse events were reported.

    Conclusions:

    • Acupuncture may offer benefits for stroke recovery, particularly in dependency and neurological function, with a good safety profile.
    • The current evidence is limited by the low quality and small sample sizes of most included trials.
    • Further large-scale, rigorously designed, multi-center randomized controlled trials are essential to validate acupuncture's role in stroke management.
