Purpose of the Study:

  • To determine the incidence, types, risk factors, and prognosis of cancer after heart transplantation.
  • To analyze long-term cancer development and outcomes in a cohort of heart transplant patients.

Main Methods:

  • A 25-year retrospective study of 541 heart transplant patients from a single center.
  • Analysis included incidence, cancer types, risk factor identification (age, gender), and survival post-diagnosis.
  • Multivariate Cox proportional hazards analysis was used to identify significant risk factors.

Main Results:

  • Cancer was diagnosed in 181 patients (33.5%) at a mean of 7.7 years post-transplant.
  • Cumulative cancer incidence was 14% at 5 years, 29% at 10 years, and 60% at 20 years.
  • Common cancers included skin cancers (squamous and basal cell), lung, lymphoma, and prostate. Age >50 and male gender were significant risk factors. Survival differed significantly between skin and non-cutaneous cancers.

Conclusions:

  • Older age and male gender are identified as key risk factors for developing post-transplant malignancy.
  • Prognosis varies greatly, with skin cancers having a much better survival rate than non-cutaneous malignancies.
