Summary

Identifying insulin resistance (IR) in ischemic cardiovascular disease patients is crucial. Sagittal abdominal diameter (SAD) showed the strongest correlation with IR in hospitalized coronary artery disease patients.

Purpose of the Study:

  • To associate anthropometric and body composition indicators with insulin resistance (IR) in patients diagnosed with coronary artery disease.
  • To evaluate the predictive value of various measurements for IR in this patient cohort.

Main Methods:

  • A cross-sectional study was conducted involving hospitalized adult and elderly patients.
  • Insulin resistance was assessed using the Homeostatic Model Assessment for Insulin Resistance (HOMA-IR).
  • Anthropometric measurements, including sagittal abdominal diameter (SAD), and body composition analysis via bioelectrical impedance (BIA) were performed.

Main Results:

  • The study included 75 patients, with a mean age of 63.75 years; 64% were elderly.
  • Metabolic syndrome (MS) was diagnosed in 65.3% of patients, with high rates of sedentary behavior (81.3%) and overweight (37.4%).
  • Insulin resistance (IR) was diagnosed in 28% of patients. SAD demonstrated the strongest correlation with HOMA-IR (r=0.476, p=0.016), followed by body fat percentage (r=0.276, p=0.016) and BMI (r=0.233, p=0.040).

Conclusions:

  • Sagittal abdominal diameter (SAD) emerged as the most significant anthropometric predictor of insulin resistance (IR) in hospitalized patients with coronary artery disease.
  • These findings highlight the utility of SAD as a simple and effective tool for identifying IR in this population.
  • Integrating SAD measurements into routine clinical practice may aid in early detection and management of metabolic complications in cardiovascular patients.
