Summary

The high prevalence of smoking among patients with Chronic Obstructive Pulmonary Disease (COPD) remains a significant issue. Effective interventions, particularly cognitive-behavioral therapy combined with varenicline, show promise in aiding smoking cessation for COPD patients.

Background:

Purpose of the Study:

  • To estimate the prevalence of smoking among individuals diagnosed with COPD.
  • To analyze the diagnostic approaches and treatment strategies employed for smokers with COPD.
  • To identify factors influencing smoking cessation in COPD patients.

Main Methods:

  • A multicenter, cross-sectional epidemiological study was conducted across 30 health centers in León province.
  • Included were 833 patients over 35 years old diagnosed and treated for COPD.
  • Data collected included demographics, smoking history (pack-years), cooximetry, nicotine dependence (visual analog scale), motivation (Fagerström test), self-efficacy, mood, previous quit attempts, cognitive-behavioral therapy (CBT), and pharmacological treatments (nicotine replacement therapy, bupropion, varenicline).

Main Results:

  • A significant proportion of COPD patients (86.67%) had a history of smoking, with an average of 35.26 years of smoking and 28.36 pack-years.
  • Active smokers diagnosed via cooximetry (35.11%) were more prevalent than self-declared active smokers (29.3%).
  • Active smokers exhibited low motivation (49.80%), high dependence (49.5%), negative attitudes (52.60%), and low mood (32.05%), with an average of 2.72 quit attempts.
  • Combined cognitive-behavioral therapy (CBT) and pharmacotherapy were used in 55.8% of cases.
  • The most effective intervention was CBT combined with varenicline, achieving a 29.86% abstinence rate.
  • Overall, 51.05% of COPD patients successfully quit smoking.

Conclusions:

  • The prevalence of smoking among COPD patients in the studied region remains unacceptably high.
  • There is a need for increased clinical engagement to mitigate the impact of smoking on COPD patient health.
  • Combined therapeutic approaches, particularly CBT with varenicline, demonstrate effectiveness in promoting smoking cessation.
