Background:

Observation:

  • A 48-year-old male presented with fever, an erythematous rash, and non-follicular pustules.
  • The patient reported a preceding painful lesion on his buttock consistent with a Loxosceles spider bite.
  • The clinical presentation and lesion morphology were compatible with a Loxosceles bite in an endemic area.

Findings:

  • The patient's score using the EuroSCAR group instrument indicated a definite diagnosis of AGEP.
  • Given the compatible Loxosceles bite and absence of other AGEP triggers, the spider bite was identified as the likely cause.
  • This case supports the emerging evidence linking Loxosceles spider bites to AGEP.

Implications:

  • This case expands the known differential diagnosis for AGEP, highlighting arthropod bites as a potential trigger.
  • It underscores the importance of considering environmental exposures, such as spider bites, in patients presenting with AGEP.
  • Further research is warranted to elucidate the mechanisms by which Loxosceles venom may induce AGEP.
