Background:

  • Gliosarcoma, a rare glioblastoma variant, exhibits glial and mesenchymal differentiation.
  • Fine-needle aspiration (FNA) is a standard diagnostic tool for various malignancies.
  • Extracranial metastases of gliosarcoma are exceptionally uncommon.

Observation:

  • A 51-year-old male presented with a posterior mediastinal mass.
  • Cytologic evaluation of the mass via FNA was performed.
  • The smears revealed a pleomorphic malignant tumor with spindled and epithelioid cells.

Findings:

  • The cytomorphologic features were consistent with gliosarcoma.
  • Histopathologic correlation confirmed the diagnosis.
  • Clinico-radiologic data supported the metastatic nature of the tumor.

Implications:

  • Fine-needle aspiration (FNA) can effectively diagnose rare extracranial gliosarcomas.
  • This case underscores the utility of FNA in challenging diagnostic scenarios.
  • Accurate diagnosis of rare tumors is crucial for appropriate patient management.
