Background:

Leuprolide acetate is a gonadotropin-releasing hormone (GnRH) agonist used for prostate cancer treatment.
Pituitary apoplexy is a rare but serious complication associated with GnRH agonist therapy.

Observation:

A 63-year-old male patient developed headache, blurred vision, and loss of consciousness following initial leuprolide acetate administration.
Neuroimaging confirmed pituitary hemorrhage in the affected patient.

Findings:

The case highlights a rare adverse event of pituitary apoplexy following leuprolide acetate initiation.
Pituitary hemorrhage occurred as a complication of GnRH agonist therapy.

Implications:

Clinicians must maintain a high index of suspicion for pituitary apoplexy in patients receiving leuprolide acetate.
