Summary

Transbronchial cryobiopsy offers a safer alternative for diagnosing diffuse lung disease, yielding larger samples than forceps biopsies. While diagnostic rates are slightly lower than surgical lung biopsy, it significantly reduces mortality risks.

Area of Science:

  • Pulmonary Medicine
  • Pathology
  • Interventional Pulmonology

Background:

  • Transbronchial cryobiopsy is emerging as a viable alternative to surgical lung biopsy for diagnosing diffuse lung diseases.
  • This technique offers potential advantages in sample size and safety compared to traditional methods.

Purpose of the Study:

  • To provide pathologists with a comprehensive understanding of transbronchial cryobiopsy.
  • To cover its methodology, comparative performance, technical challenges, complications, interpretation, and clinical utility in diffuse lung disease diagnosis.

Main Methods:

  • A literature review of transbronchial cryobiopsy in diffuse lung disease up to May 2016 was conducted.
  • Emphasis was placed on studies from the last five years, supplemented with author-known unpublished data.

Main Results:

  • Cryobiopsies yield significantly larger tissue samples compared to forceps biopsies, enabling pattern recognition similar to surgical lung biopsies.
  • Artifacts are minimal, and diagnostic rates approach 80%, slightly lower than surgical lung biopsies (over 90%).

Conclusions:

  • Transbronchial cryobiopsy is a promising alternative to surgical lung biopsy, potentially reducing the need for more invasive procedures.
  • It offers a significantly lower mortality rate (0.1%) compared to surgical lung biopsy (1.7% or higher).
