Background:

  • Previous shunt revisions and anatomical challenges necessitate alternative strategies.

Observation:

  • An 18-year-old male with prematurity-related hydrocephalus and over 30 shunt revisions presented with shunt malfunction.
  • Peritoneal and pleural spaces were unsuitable for distal shunt placement due to prior use.
  • Subclavian vein thrombosis precluded standard atrial cannulation for shunt placement.

Findings:

  • A direct ventricle to heart shunt was successfully implanted, bypassing venous access issues.
  • Postoperative catheter migration required repeat surgery for direct atrial reimplantation.
  • The shunt functioned successfully for 16 months without complications like pericardial effusion.

Implications:

  • Direct ventricle to heart shunt placement is a viable, albeit complex, option for select hydrocephalus patients.
  • This technique addresses challenges posed by venous thrombosis and exhausted distal shunt sites.
  • Further research into the long-term efficacy and safety of this approach is warranted.
