  • Spirometry, cardiovascular, and metabolic measures were collected.
  • All-cause and cardiovascular mortality served as endpoints.

  • Univariate analysis showed both FEV1 and FVC predicted mortality.
  • Multivariate analysis revealed FVC independently predicted cardiovascular mortality, even with C-reactive protein (CRP) considered.
  • The predictive significance of FVC was lost when CRP was replaced by interleukin-6.

  • Forced vital capacity (FVC), not FEV1, is a significant predictor of all-cause and cardiovascular mortality in black South Africans.
  • Inflammation may mediate the relationship between FVC and mortality.
