Bulimia nervosa (BN) patients exhibit altered brain activity in attention networks, impacting alerting, reorienting, and executive functions. These neural differences may contribute to comorbid ADHD symptoms and eating disorder severity.

Background:

  • Bulimia nervosa (BN) is increasingly linked to comorbid attention-deficit/hyperactivity disorder (ADHD) and attentional deficits.
  • The neural underpinnings of these attentional impairments in BN remain largely unexplored.

Purpose of the Study:

  • To investigate the neural mechanisms of attentional functions in BN patients using functional magnetic resonance imaging (fMRI).
  • To examine how attention networks (alerting, reorienting, executive attention) are differentially affected in BN.

Main Methods:

  • fMRI study involving 20 BN patients and 20 healthy controls (HC).
  • Utilized a modified Attention Network Task (ANT) to assess neural activity across three attention networks.
  • Analyzed brain activation patterns and their correlation with clinical symptoms (ADHD, impulsivity, eating disorder severity).

  • BN patients displayed hyperactivation in parieto-occipital regions and reduced default-mode network (DMN) deactivation during alerting.

  • BN is associated with widespread alterations across all three major attention networks.
  • Dysfunctional alerting and DMN deactivation may relate to preoccupations with food or body image.
  • Hypoactivation in executive control and TPJ networks could underlie inattentive, impulsive behaviors, and poor emotion regulation.
  • Attentional network dysfunction in BN is complex, extending beyond executive attention, and warrants consideration in diagnosis and treatment.
