Background:

  • Cystic echinococcosis is a parasitic zoonosis endemic in the Mediterranean area.
  • While liver and lung are common sites, splenic hydatid disease is rarer, with other locations being exceptional.
  • This study focuses on rare cases of hydatid disease outside the liver and lungs.

Purpose of the Study:

  • To report on the experience with extrahepatic and extrapulmonary hydatidosis.
  • To analyze diagnostic and therapeutic approaches for these rare presentations.

Main Methods:

  • Retrospective review of 18 patients with extrahepatic/extrapulmonary hydatid disease from May 2007 to December 2014.
  • Utilized medical records, serology, ultrasound, and CT scans for diagnosis.
  • Evaluated surgical and non-surgical treatment outcomes.

  • 18 patients (13% of total hydatid disease cases) presented with extrahepatic/extrapulmonary disease.
  • Mean age was 44.5 years; 50% had concurrent hepatic echinococcosis.
  • Surgery was performed on 13 patients, with 12 radical cystectomies; no recurrences were noted.

Conclusions:

  • Closed total cystectomy is the preferred surgical treatment to prevent recurrence, except for splenic hydatid disease where splenectomy is recommended.
  • Conservative techniques are suitable for multiple hydatid disease or high-risk patients.
  • Effective management of rare hydatid disease locations is achievable with appropriate surgical strategies.
