Drugs for Treatment of Constipation-Predominant IBS

1.1K
Pharmacological therapies for IBS-C are designed to alleviate abdominal discomfort and enhance bowel function. In patients with IBS-C, fiber supplements may help soften stools and decrease straining, but may also lead to increased gas production and bloating. Osmotic laxatives like milk of magnesia are frequently used to soften stools and increase stool frequency in IBS-C patients. In addition, two drugs approved for use in severe IBS-C adult cases are linaclotide (Linzess) and lubiprostone...

Summary

Electroacupuncture (EA) significantly increased complete spontaneous bowel movements (CSBMs) in patients with chronic severe functional constipation (CSFC). This 8-week treatment is safe and effective for improving constipation symptoms.

Area of Science:

  • Gastroenterology
  • Integrative Medicine
  • Clinical Trials

Background:

  • Acupuncture is explored for chronic constipation, yet robust evidence is limited.
  • Chronic severe functional constipation (CSFC) affects a significant patient population.
  • The efficacy of electroacupuncture (EA) for CSFC requires further investigation.

Purpose of the Study:

  • To evaluate the effectiveness of electroacupuncture (EA) in treating chronic severe functional constipation (CSFC).
  • To compare EA with sham acupuncture (SA) in a randomized controlled trial setting.

Main Methods:

  • A randomized, parallel, sham-controlled trial involving 1075 patients across 15 hospitals in China.
  • Participants received 28 sessions of either EA at traditional acupoints or SA at nonacupoints over 8 weeks.
  • The primary outcome was the change in mean weekly complete spontaneous bowel movements (CSBMs) from baseline during weeks 1-8, with follow-up until week 20.

Main Results:

  • EA significantly increased mean weekly CSBMs compared to SA during both the treatment (1.76 vs 0.87, P < 0.001) and follow-up (1.96 vs 0.89, P < 0.001) periods.
  • A higher proportion of patients in the EA group achieved 3 or more mean weekly CSBMs (31.3% vs 12.1% during treatment, 37.7% vs 14.1% during follow-up; P < 0.001).
  • Adverse events related to acupuncture were infrequent and mild/transient in both groups.

Conclusions:

  • Eight weeks of EA is a safe and effective treatment for increasing CSBMs in patients with CSFC.
  • EA demonstrates significant therapeutic benefits for chronic constipation.
  • Further research is recommended to assess longer-term treatment and follow-up durations.
