Background:

Purpose of the Study:

  • To introduce and validate a novel quadrant field measure of the melanopsin retinal ganglion cell-mediated pupil light response.
  • To assess the diagnostic accuracy of the superonasal quadrant post-illumination pupil response (PIPR) in differentiating glaucoma suspects and early glaucoma patients from healthy controls.

Main Methods:

  • Measured the melanopsin-mediated post-illumination pupil response (PIPR) in the superonasal quadrant.
  • Enrolled 46 patients with varying stages of glaucoma (including glaucoma suspects) and 21 healthy participants.
  • Compared PIPR measurements between patient groups and the control group.

Main Results:

  • The superonasal quadrant PIPR demonstrated fair diagnostic accuracy (AUC=0.74) for glaucoma suspects and excellent accuracy (AUC=0.94) for early glaucoma patients.
  • Superonasal PIPR showed a linear correlation with retinal nerve fiber layer thinning in glaucoma suspects and early glaucoma.
  • Melanopsin dysfunction was detected in early glaucoma patients at pre-perimetric stages.

Conclusions:

  • Quadrant PIPR stimulation is a promising method for detecting melanopsin dysfunction in early glaucoma and pre-perimetric stages.
  • This novel functional test may aid in early diagnosis and treatment decisions for glaucoma suspects.
  • The superonasal PIPR serves as a sensitive indicator of visual function deficits in early glaucoma.
