Robot-assisted laparoscopic partial nephrectomy (RLPN) successfully treated renal cell carcinoma in a pregnant patient. This minimally invasive approach offers a safe and feasible option for managing kidney cancer during pregnancy.

Area of Science:

  • Urology
  • Surgical Oncology
  • Maternal-Fetal Medicine

Background:

  • Renal cell carcinoma (RCC) during pregnancy is rare.
  • Surgical management of RCC in pregnant patients presents unique challenges.
  • Minimally invasive techniques are increasingly explored for complex surgical conditions during gestation.

Purpose of the Study:

  • To report the first case of robot-assisted laparoscopic partial nephrectomy (RLPN) performed during pregnancy.
  • To evaluate the safety and feasibility of RLPN in a pregnant patient with RCC.
  • To highlight the potential of robotic surgery in managing oncologic conditions during gestation.

Main Methods:

  • A 36-year-old pregnant woman underwent RLPN at 14 weeks gestational age for T1a conventional-type RCC.

  • The procedure utilized the da Vinci™ surgical system.
  • Key operative metrics including operative time, warm ischemia time, and estimated blood loss were recorded.
  • Main Results:

    • The RLPN was completed successfully with a total operative time of 165 minutes.
    • Warm ischemia time was 28 minutes, and estimated blood loss was 100 ml.
    • Pathologic evaluation confirmed T1a RCC, Fuhrman nuclear grade 3, with negative margins. The pregnancy proceeded uneventfully.

    Conclusions:

    • Robot-assisted laparoscopic partial nephrectomy is a safe and feasible surgical option for pregnant patients with renal cell carcinoma.
    • This case represents the first successful application of robotic surgery for partial nephrectomy during pregnancy.
    • Minimally invasive robotic surgery may offer advantages over traditional open or laparoscopic approaches in this specific patient population.
