    Background:

    • Simultaneous bilateral rhegmatogenous retinal detachments present unique clinical challenges.
    • Previous studies primarily focused on scleral buckle (SB) for management.

    Purpose of the Study:

    • To evaluate the clinical course and outcomes of simultaneous bilateral retinal detachments treated with contemporary vitreoretinal surgical methods.
    • To compare the efficacy of different surgical approaches.

    Main Methods:

    • Retrospective review of 21 patients (42 eyes) diagnosed with simultaneous bilateral rhegmatogenous retinal detachments.
    • Analysis of surgical techniques including scleral buckle (SB) alone, pars plana vitrectomy (PPV) alone, and combined PPV/SB.

    Main Results:

    • Mean visual acuity improved from 20/100 to 20/40 post-operatively (P = .01).
    • High primary retinal reattachment rate of 98% was achieved.
    • Final visual acuity varied by surgical method: SB (20/30), PPV (20/40), and combined PPV/SB (20/80), with statistical significance (P = .019).

    Conclusions:

    • Multiple surgical modalities, including vitrectomy, are effective for simultaneous bilateral retinal detachments.
    • High success rates in retinal reattachment were observed irrespective of the chosen surgical technique.
    • Contemporary surgical management offers favorable outcomes for this condition.
