    Background:

    • Predicting 30-day readmissions remains a challenge in academic medical centers.

    Purpose of the Study:

    • To evaluate the utility of clinician judgment in predicting 30-day hospital readmissions.
    • To determine if clinicians' subjective assessment of surprise regarding a patient's readmission can identify high-risk individuals.

    Main Methods:

    • Prospective interviews with clinicians regarding their surprise if a patient was readmitted within 30 days.
    • Identification of one highest-risk patient by clinicians.
    • Multivariate models to assess the impact of clinician judgment on readmission.

    Main Results:

    • The 30-day same-hospital readmission rate was 10.7%.
    • Factors associated with readmission included the number of hospitalizations (OR, 1.16; 95% CI, 1.04-1.30), emergency department visits (OR, 1.10; 95% CI, 1.02-1.19), and discharge medications (OR, 1.07; 95% CI, 1.00-1.14).
    • The negative-predictive value of clinician surprise was high (95%).

    Conclusions:

    • Clinicians demonstrated a higher accuracy in identifying patients who would *not* be readmitted.
    • Clinician judgment, particularly their sense of surprise, can be a valuable tool in identifying patients at lower risk of readmission.
