Summary

Anterolateral rotatory instability after anterior cruciate ligament reconstruction can persist. This study presents a technique for anatomic anterolateral ligament reconstruction to address this residual knee instability.

Area of Science:

  • Orthopedic Surgery
  • Sports Medicine
  • Knee Biomechanics

Background:

  • Anterior cruciate ligament (ACL) reconstruction effectively restores anteroposterior stability.
  • Anterolateral rotatory instability remains a challenge in a subset of ACL-reconstructed patients.
  • The anterolateral ligament (ALL) is increasingly recognized for its role in knee stability.

Purpose of the Study:

  • To describe a surgical technique for anatomic ALL reconstruction.
  • To address residual anterolateral rotatory instability after ACL injury.
  • To highlight the functional importance of the ALL in knee biomechanics.

Main Methods:

  • Anatomic ALL reconstruction technique detailed.
  • Utilizes a semitendinosus tendon allograft.
  • Focuses on restoring rotational knee stability.

Main Results:

  • The described technique aims to improve anterolateral rotatory stability.
  • Addresses a specific deficit not fully corrected by traditional ACL reconstruction.
  • Provides a surgical option for patients with persistent ALL deficiency.

Conclusions:

  • Anatomic ALL reconstruction is a viable option for residual anterolateral rotatory instability.
  • Surgical techniques addressing lateral-sided structures are crucial for knee stability.
  • Further research into ALL function and reconstruction is warranted.
