Summary

Elderly patients with chronic rhinosinusitis with nasal polyps (CRSwNP) experienced lower recurrence rates after surgery compared to young adults. This suggests different management strategies may be needed for older CRSwNP patients.

Background:

  • Chronic rhinosinusitis with nasal polyps (CRSwNP) management in elderly patients requires specific consideration.
  • Functional endoscopic sinus surgery (FESS) is a key treatment modality for CRSwNP.

Purpose of the Study:

  • To evaluate FESS outcomes for CRSwNP in elderly patients (≥65 years).
  • To identify prognostic factors for CRSwNP recurrence in the elderly.
  • To compare CRSwNP characteristics between elderly and young adult patients (20-40 years).

Main Methods:

  • Retrospective review of 43 elderly and 71 young adult CRSwNP patients undergoing FESS.
  • Analysis of demographic, clinical, laboratory, histologic, and prognostic data.
  • Comparison of recurrence rates and associated factors between age groups.

Main Results:

  • Elderly patients showed significantly lower CRSwNP recurrence rates post-FESS compared to young adults (p=0.05).
  • In elderly patients, recurrence was associated with allergy, asthma, and ASA intolerance.
  • Young adults had recurrence linked to asthma, ASA intolerance, and higher serum eosinophil counts, with eosinophilic CRSwNP showing higher recurrence (p=0.001).

Conclusions:

  • Elderly CRSwNP patients have a lower recurrence rate after FESS than younger adults.
  • Prognostic factors for recurrence differ between elderly and young adult CRSwNP patients.
  • Eosinophilic CRSwNP's high recurrence risk may not apply to elderly patients, warranting further investigation.
