Background:

Primary fascial closure after damage control laparostomy is challenging.
Existing devices lack a clear best practice.
The Abdominal Reapproximation Anchor (ABRA) device was introduced at Westmead Hospital in July 2013.

Observation:

A retrospective review identified four patients managed with the ABRA device.
Indications included intra-abdominal sepsis (3 patients) and penetrating trauma (1 patient).
Mean BMI was 33.5 kg/m², APACHE II score was 14.5.

Three out of four patients (75%) achieved successful fascial closure.
