    Background:

    • Verruciform xanthoma (VX) is a rare benign oral and cutaneous lesion.
    • Previous reports describe its characteristic clinical and microscopic features.

    Observation:

    • The 26th reported case of verruciform xanthoma is presented.
    • The lesion, measuring 1.5 by 4 cm, is the largest VX documented to date.
    • This case represents the first report of VX in a Black individual.

    Findings:

    • Clinical presentation showed a shaggy, parakeratin-covered, verrucous surface.
    • Microscopic examination revealed elongated rete pegs extending into connective tissue.
    • Abundant large 'foam cells' (xanthoma cells) were observed within the connective tissue papillae.

    Implications:

    • This case expands the known demographic and size spectrum of verruciform xanthoma.
    • Reinforces the diagnostic criteria for verruciform xanthoma in dermatopathology.
    • Highlights the importance of considering diverse patient populations in the diagnosis of skin lesions.
