Summary

Clobetasol propionate ointment can cause milia, small cysts, in patients with chronic skin conditions. Histologic analysis confirmed these milia originate from eccrine glands after two weeks of use.

Background:

  • Clobetasol propionate is a potent topical corticosteroid used for various dermatoses.
  • Adverse effects of topical corticosteroids are well-documented, but specific associations with milia require further investigation.

Observation:

  • Two patients with distinct chronic dermatoses developed milia.
  • Milia appeared within two weeks of initiating treatment with clobetasol propionate ointment.

Findings:

  • Histologic examination revealed the milia to be epidermal inclusion cysts.
  • Immunohistochemical analysis confirmed the origin of these cysts from eccrine glands.

Implications:

  • This finding suggests a potential iatrogenic cause of milia related to clobetasol propionate therapy.
  • Clinicians should be aware of this possible side effect when prescribing potent topical corticosteroids for chronic skin conditions.
