Cardiac dysfunction in type II diabetes: a bittersweet, weighty problem, or both?

Weight loss and improved glycemic control in type 2 diabetes patients significantly benefit heart function. Combining both approaches yields additive improvements in left ventricular systolic and diastolic function.

Background:
- Obesity and type 2 diabetes are linked to impaired left ventricular (LV) function.
- Weight loss is known to improve LV function, but the independent and combined effects of weight loss and glycemic control on LV function in type 2 diabetes remain unclear.
Purpose of the Study:
- To investigate the incremental impact of weight loss and improved glycemic control on left ventricular (LV) function in patients with type 2 diabetes.
- To determine if benefits on LV function are mediated by weight loss or glycemic control improvements.
Main Methods:
- A 12-month case-control study involving three groups of type 2 diabetes patients with suboptimal glycemic control.
- Group 1: No improvement in HbA1c or weight loss. Group 2: Improved HbA1c (≥1%) but no weight loss. Group 3: Improved HbA1c (≥1%) and weight loss.
- Transthoracic echocardiograms were performed at baseline and follow-up to assess LV function, including global longitudinal strain (GLS).
Main Results:
- At baseline, Group 3 had the highest BMI and worst LV systolic function (GLS).
- After 12 months, both Group 2 and Group 3 showed improvements in LV ejection fraction and diastolic function, with reduced filling pressures.
- Group 3 exhibited more significant improvements in GLS compared to Group 2, with both groups achieving comparable GLS at follow-up.
Conclusions:
- In overweight patients with type 2 diabetes, weight loss and improved glycemic control have additive beneficial effects on LV systolic and diastolic function.
- These findings suggest a combined strategy of weight management and glycemic control is optimal for cardiovascular health in this population.
