Summary

An hemostatic matrix aids hemostasis during laparoscopic partial nephrectomy (LPN), reducing bleeding and complications. This retrospective study suggests its benefit, but further prospective research is needed for confirmation.

Background:

  • Laparoscopic partial nephrectomy (LPN) is a standard procedure for kidney tumors.
  • Optimizing hemostasis and minimizing perioperative complications are crucial in LPN.

Purpose of the Study:

  • To evaluate the efficacy of an hemostatic matrix in improving hemostasis during LPN.
  • To assess the impact of hemostatic matrix use on perioperative outcomes and complications in LPN patients.

Main Methods:

  • Retrospective analysis of 85 patients undergoing LPN, divided into two groups: with (Group 1, n=41) and without (Group 2, n=44) hemostatic matrix use.
  • Comparison of demographic data, tumor characteristics (RENAL score, size), operative parameters (operation time, ischemia time), and perioperative outcomes (hemorrhage, transfusion, complications).

Main Results:

  • Patients using hemostatic matrix (Group 1) had statistically higher mean tumor size and RENAL nephrometry scores.
  • Group 1 demonstrated significantly less hemorrhage, reduced blood transfusion requirements, and shorter ischemia and operation times.
  • Pelvicaliceal repair was more frequent in Group 1 due to deeper tumor extension.

Conclusions:

  • Adjunctive use of a hemostatic matrix appears to improve hemostasis and decrease hemorrhagic complications in LPN.
  • Further prospective studies are warranted to definitively establish the role of hemostatic matrix in LPN procedures.
