    Summary

    Area of Science:

    • Nephrology
    • Oncology
    • Pathology

    Background:

    • Nephrotic syndrome is a kidney disorder often associated with various cancers, including lymphoproliferative diseases and solid tumors.
    • Adenocarcinoma of the Vater ampulla is a rare malignancy of the digestive system.

    Observation:

    • A patient presented with simultaneous symptoms of nephrotic syndrome and adenocarcinoma of the Vater ampulla.
    • Renal biopsy showed membranous glomerulonephritis with characteristic subepithelial deposits.

    Findings:

    • The subepithelial deposits stained positive for IgG and C'3 in a granular pattern.
    • No evidence of CEA-antiCEA immune complexes was found in the glomerular capillaries.
    • This case represents the first documented association between adenocarcinoma of the Vater ampulla and nephrotic syndrome.

    Implications:

    • This finding suggests a potential paraneoplastic link between Vater ampulla adenocarcinoma and membranous glomerulonephritis.
    • Further research is needed to elucidate the underlying mechanisms connecting this specific cancer and nephrotic syndrome.
    • Clinicians should consider screening for Vater ampulla adenocarcinoma in patients presenting with unexplained nephrotic syndrome.
