Anti-cyclic citrullinated peptide (anti-CCP) antibodies, often specific to rheumatoid arthritis, can be falsely positive in tuberculosis patients. This study found higher anti-CCP positivity in pulmonary tuberculosis than extra-pulmonary TB.

- Anti-CCP antibodies were detected in 38.7% of pulmonary TB patients and 10.52% of extra-pulmonary TB patients.
- Within pulmonary TB, sputum-positive cases showed higher anti-CCP positivity (55.56%) compared to sputum-negative cases (31.81%).
- Overall, 12 out of 31 pulmonary TB patients and 2 out of 17 extra-pulmonary TB patients tested positive for anti-CCP antibodies.

- Tuberculosis, particularly pulmonary TB, can lead to false-positive results for anti-CCP antibodies.
- The specificity of anti-CCP antibodies for rheumatoid arthritis may be compromised in patients with active tuberculosis.
- Further investigation is warranted to understand the immunological mechanisms underlying anti-CCP positivity in TB.
