Background:

  • Vesicoureteral reflux (VUR) is a common condition in children.
  • The reliability of voiding cystourethrogram (VCUG) for diagnosing VUR requires further evaluation.

Purpose of the Study:

  • To assess the reliability of VCUG in detecting and grading vesicoureteral reflux in pediatric patients.
  • To determine if repeat bladder fillings and voidings improve diagnostic accuracy.

Main Methods:

  • Prospective evaluation of 207 children undergoing VCUG.
  • Each child had two or more bladder fillings and voidings during the procedure.
  • Discrepancies in VUR presence and grade were analyzed between cycles.

Main Results:

  • A 12% discrepancy in VUR presence or grade was observed with two cycles and 20% with three cycles.
  • Most changes occurred between minor grades of VUR.
  • Test agreement was 64% for abnormal findings and 96% for normal findings on the first voiding cycle.

Conclusions:

  • VCUG is more reliable for ruling out VUR than for grading its severity.
  • A second voiding cycle can enhance the diagnostic reliability of VCUG for VUR.
  • The test is highly reliable for documenting the absence of VUR.
