Summary
This study presents the first case of spinal pseudarthrosis in ochronosis, demonstrating posterior-only 360° surgery for ankylosed spine fusion. This approach offers an effective alternative for managing spinal pseudarthrosis.

- At 3 months, the patient could walk independently.

- Ochronosis should be considered an etiology for spinal pseudarthrosis.
- Suspect pseudarthrosis in patients with ankylosing diseases experiencing worsening back pain or neurological deficits.
- Posterior-only 360° surgery may be a viable alternative to combined anterior-posterior approaches for spinal pseudarthrosis.
