Rituximab in Lymphoma and Chronic Lymphocytic Leukaemia: A Practice Guideline

Background:

• Rituximab was the first monoclonal antibody approved by the FDA for cancer treatment.

Purpose of the Study:

• To systematically review the literature on rituximab in lymphoma and CLL.
• To provide evidence-based consensus guidelines for the rational use of rituximab.
• To update current clinical practice recommendations.

Main Methods:

• Validated methodology from the Cancer Care Ontario Program in Evidence-based Care was employed.
• A comprehensive literature search identified relevant randomized controlled trials (RCTs).
• Data from 56 primary RCTs of rituximab-containing chemotherapy regimens were extracted.

Main Results:

• Rituximab addition to chemotherapy improved progression-free and overall survival in aggressive B-cell lymphomas (including DLBCL, Burkitt, HIV-related lymphoma).
• Benefits were observed in initial and subsequent treatment of follicular and other indolent B-cell lymphomas.
• Rituximab maintenance improved outcomes in indolent lymphomas; it also benefited initial CLL treatment with fludarabine or chlorambucil.

Conclusions:

• Rituximab is recommended for the initial treatment of aggressive B-cell lymphomas.
• Recommended for initial and subsequent treatment of indolent B-cell lymphomas, and as maintenance therapy.
• Recommended for the initial treatment of chronic lymphocytic leukemia (CLL).
