Summary

Heparin therapy may offer a novel treatment for nephrotic syndrome in children unresponsive to standard therapies. This approach led to reduced proteinuria and sustained remission in a young patient, suggesting potential long-term benefits.

Area of Science:

  • Pediatric Nephrology
  • Glomerular Diseases

Background:

  • Focal glomerulosclerosis (FGS) is a significant cause of nephrotic syndrome in children.
  • Treatment resistance to corticosteroids and cyclophosphamide presents a therapeutic challenge.

Observation:

  • A pediatric patient with FGS and nephrotic syndrome demonstrated unresponsiveness to standard immunosuppressive therapies.
  • Prolonged subcutaneous heparin administration was initiated as an alternative treatment strategy.

Findings:

  • Subcutaneous heparin resulted in a notable reduction in proteinuria.
  • The patient regained sensitivity to corticosteroids.
  • Histological examination showed no further deterioration, with possible improvement observed.
  • The patient remained clinically stable and well for five years post-treatment.

Implications:

  • Subcutaneous heparin may represent a viable therapeutic option for steroid- and cyclophosphamide-resistant nephrotic syndrome in pediatric focal glomerulosclerosis.
  • This case highlights the potential of anticoagulation therapy in managing complex glomerular diseases.
  • Further research is warranted to explore the efficacy and mechanisms of heparin in treating resistant nephrotic syndrome.
