Summary

The supra-auricular approach for preauricular sinus excision shows significantly lower recurrence rates and fewer complications compared to tract sinusectomy. Surgeons should consider the supra-auricular approach as a standard procedure for better patient outcomes.

Area of Science:

  • Otolaryngology
  • Plastic Surgery
  • Surgical Innovation

Background:

  • Preauricular sinus excision has a high recurrence rate, necessitating improved surgical techniques.
  • Existing methods like tract sinusectomy have limitations in preventing recurrence and causing complications.

Approach:

  • A systematic literature review was conducted across major databases (LILACS, MEDLINE, SciELO, PubMed, Cochrane Library).
  • 17 studies encompassing 1270 preauricular sinuses were analyzed, comparing sinusectomy and supra-auricular approaches.
  • Recurrence rates, complications, and facial nerve integrity were key outcome measures.

Key Points:

  • The supra-auricular approach demonstrated a significantly lower recurrence rate (1.3%) compared to tract sinusectomy (8.1%).
  • Sinusectomy was associated with a significantly higher incidence of complications.

  • No cases of facial nerve paresis or paralysis were reported for either surgical approach.
  • Conclusions:

    • The supra-auricular approach is superior to tract sinusectomy for preauricular sinus excision due to reduced recurrence.
    • This approach should be considered the standard procedure for preauricular sinus excision.
    • Familiarity with the supra-auricular approach is recommended for surgeons to optimize patient care.
