• ¹Cheri Lattimer, BSN, RN, serves as the Executive Director for the National Transitions of Care Coalition (NTOCC) and Chief Policy Officer for the Case Management Society of America. Her leadership in public policy, quality improvement, case management, care coordination, and transitions of care is recognized on the national and international landscape. She serves on several national boards and committees, URAC, Roundtable on Critical Care Policy, CMS Caregiver Workgroup, American Nurses Association, and Care Coordination Measures Workgroup; and participates on several of the Society of Hospital Medicine Advisory Committees, Diabetes, Heart Failure, Hospital Collaborative Care, and Better Outcomes by Optimizing Safe Transitions.
