A patient developed unusual eyelid movements on the opposite side of an injury to the right third cranial nerve. This case is the first to describe a pseudo-Graefe phenomenon contralateral to a regenerating third nerve injury.

Area of Science:

  • Ophthalmology
  • Neuroscience
  • Neurology

Background:

  • Third nerve paresis, often caused by trauma, affects eye movement and eyelid position.
  • Oculomotor synkinesis involves unintended movements of one muscle group associated with the movement of another.

Observation:

  • A patient with traumatic right third nerve paresis developed contralateral oculomotor synkinesis affecting the left upper eyelid.
  • The left upper eyelid elevated with infraduction in adduction of the nonparetic left eye.
  • Right upper eyelid elevation occurred in adduction of the paretic right eye but was absent in downgaze.

Findings:

  • This case presents the first documented instance of a pseudo-Graefe phenomenon occurring contralateral to a regenerating paretic third nerve.
  • The observed synkinesis demonstrates complex neural pathway reorganization following nerve injury.

Related Experiment Videos

Implications:

  • Understanding such contralateral synkinesis can improve diagnosis and management of third nerve palsies.
  • This phenomenon offers insights into neural plasticity and aberrant regeneration pathways in the oculomotor system.
