Anterior Cingulate Implant for Obsessive-Compulsive Disorder.

Background:

• Obsessive-Compulsive Disorder (OCD) affects 2.3% of the population, causing significant distress and functional impairment.

Observation:

• A novel approach involved bilateral electrode implantation in the rostrodorsal anterior cingulate cortex (ACC) for a patient with severe OCD and alcohol addiction.
• The patient's alcohol use stemmed from self-treating underlying OCD, anxiety, and depression.

Findings:

• Deep brain stimulation of the dorsal ACC resulted in a 65.5% decrease in OCD symptoms and an 87.5% reduction in alcohol craving after 36 weeks.
• While symptom scores improved, the reduction in actual weekly alcohol consumption was modest (50 to 32 units).

• These preliminary findings suggest that anterior cingulate cortex stimulation may be a promising treatment for intractable OCD.
