Background:

Purpose of the Study:

  • To analyze the outcomes of testis-sparing surgical procedures in boys diagnosed with Leydig cell pathologies.
  • To evaluate the efficacy and safety of conservative surgical management for pediatric Leydig cell tumors.

Main Methods:

  • Retrospective analysis of four boys with Leydig cell hyperplasia and one with Leydig cell tumor who underwent testis-sparing surgery.
  • Preoperative evaluation included scrotal ultrasonography, tumor markers (β-human chorionic gonadotropin, α-fetoprotein), and hormonal profiles.
  • Surgical technique involved tumor enucleation followed by testicular repositioning and fixation.

Main Results:

  • All patients presented with testicular masses and scrotal asymmetry; ultrasonography was key in diagnosis.
  • Three patients had Leydig cell hyperplasia, and one had a Leydig cell tumor; all had precocious puberty signs.
  • Follow-up averaged 4.8 years with no recurrence or testicular atrophy; one patient required successful contralateral lesion enucleation.

Conclusions:

  • Testis-sparing surgery is a viable and effective treatment for pediatric Leydig cell pathologies, offering significant advantages.
  • The procedure is technically feasible with good long-term functional and cosmetic results.
  • Conservative management is recommended for patients with salvageable testicular tissue and normal tumor markers.
