[Surgery of traumatic aortic rupture].

L von Segesser, K Schneider, R Siebenmann

    Helvetica Chirurgica Acta
    |June 1, 1989

    Summary

This study on acute traumatic aortic rupture found that direct aortic cross-clamping without spinal cord protection was safe and effective. No paraplegia occurred in survivors, indicating a viable surgical approach.

    Area of Science:

    • Cardiovascular Surgery
    • Trauma Surgery
    • Thoracic Surgery

    Context:

    • Acute traumatic rupture of the aorta is a life-threatening injury.
    • Surgical repair is complex and carries significant risks, including spinal cord injury.
    • Previous techniques often involved complex spinal cord protection methods.

    Purpose:

    • To report the clinical presentation, diagnosis, surgical techniques, and outcomes of patients with acute traumatic aortic rupture.
    • To evaluate the safety and efficacy of direct aortic cross-clamping without additional spinal cord protection.
    • To assess the incidence of neurological complications in survivors.

    Summary:

    • Twenty-one consecutive patients with acute traumatic aortic rupture underwent surgical repair.
    • Direct aortic cross-clamping was employed in 18 patients (86%), with direct suture in 12 (60%) and Dacron graft in the remainder.
    • Overall mortality was 33% (7/21), with causes including polytrauma, septicemia, and myocardial infarction. Importantly, no paraplegia or paraparesis occurred in surviving patients.

    Impact:

    • Direct aortic cross-clamping is a feasible and effective method for repairing acute traumatic aortic rupture.
    • This technique can be performed without additional spinal cord protection, reducing surgical complexity.
    • The study demonstrates a low incidence of spinal cord injury in survivors, suggesting improved patient outcomes.
