Severe cholestasis can cause acute kidney injury (AKI) through bile cast nephropathy. Prompt treatment of biliary obstruction led to complete kidney recovery in a patient with obstructive cholestasis.
Area of Science:
- Nephrology
- Hepatology
- Gastroenterology
Background:
- Acute kidney injury (AKI) is a common complication in liver disease patients.
- Hepatorenal syndrome is a known cause, but bile cast nephropathy is less understood.
- Renal histology data in severe liver failure is limited.
Observation:
- A case of bile cast nephropathy in a patient with obstructive cholestasis due to common bile duct stones.
- Kidney biopsy revealed tubular bile casts, tubular injury, and bilirubin composition.
- The patient had no other identifiable cause for kidney failure.
Findings:
- Severe cholestasis alone can precipitate AKI.
- Bile cast nephropathy is characterized by tubular bile cast formation.
- Hall stain confirmed bilirubin composition of the casts.
Implications:
- AKI in severe cholestasis is potentially reversible.
- Treatment of biliary obstruction can lead to complete kidney recovery.
- Highlights the importance of renal histology in diagnosing AKI in liver disease.
