Background:

Free-flap mandibular reconstruction is a specialized surgical procedure.
This surgery is associated with significant complications, re-interventions, and hospitalizations, leading to high costs.

Purpose of the Study:

To assess the direct hospital costs associated with osseocutaneous free-flap surgery.
To identify the primary cost drivers in this complex reconstructive procedure.

Main Methods:

A multicentric prospective micro-costing study was conducted.
Direct medical costs were evaluated from a hospital perspective, from initial consultation to patient discharge.

Main Results:

The mean total cost for free-flap intervention was 34,009€.
Hospital bed occupation was the most significant cost factor (30-90% of total cost).
Complications increased the mean cost by 77.3%, mainly due to ICU and conventional unit stays.

Conclusions:

Free-flap mandibular reconstruction is effective but highly complex and costly.
Cost management strategies should focus on optimizing hospital bed utilization and minimizing complications.
