  • Benign prostatic pathologies can clinically mimic prostate adenocarcinoma.
  • Xanthogranulomatous prostatitis is a rare, benign inflammatory condition of the prostate.

Observation:

  • A 47-year-old male presented with a 3-year history of lower urinary tract symptoms and a palpable hypogastric tumor.
  • Digital rectal examination revealed a solid prostate of approximately 60g with an initial PSA of 0.90 ng/mL.

Findings:

  • Surgical excision and prostatectomy were performed.
  • Histopathology confirmed xanthogranulomatous prostatitis, ruling out malignancy.

Implications:

  • Xanthogranulomatous prostatitis is an extremely rare entity that can simulate prostate adenocarcinoma.
  • Correct histopathological diagnosis is essential for appropriate patient management and treatment planning.
