Summary

Chronic tonsillitis is linked to higher systemic matrix metalloproteinase (MMP)-8. Patients with tonsillar cancer show elevated MMP-8 and tissue inhibitor of metalloproteinase (TIMP)-1 levels compared to those with benign tonsillar disease.

Background:

  • Low-grade systemic inflammation contributes to chronic diseases like coronary heart disease.
  • Chronic tonsillitis's long-term health impacts are not well-documented.
  • Systemic matrix metalloproteinase (MMP)-8 and tissue inhibitor of metalloproteinase (TIMP)-1 are implicated in inflammatory conditions and cancer prognosis.

Purpose of the Study:

  • To investigate systemic MMP-8 and TIMP-1 levels in patients with benign tonsillar disease and tonsillar squamous cell carcinoma.
  • To explore the association between chronic tonsillitis and systemic MMP-8 response.
  • To determine the correlation between these biomarkers and human papillomavirus (HPV) status in tonsillar cancer.

Main Methods:

  • Analyzed serum MMP-8 (S-MMP-8) using immunofluorometric assay and serum TIMP-1 (S-TIMP-1) using immunosorbent assay.
  • Included 175 patients undergoing tonsillectomy for benign tonsillar disease and 33 control patients with tonsillar squamous cell carcinoma.
  • Determined tonsillar HPV status via PCR.

Main Results:

  • Chronic tonsillitis without hypertrophy was associated with an enhanced systemic MMP-8 response.
  • Patients with tonsillar squamous cell carcinoma exhibited significantly higher S-MMP-8 and S-TIMP-1 concentrations than those with benign tonsillar disease.
  • No correlation was found between S-MMP-8 or S-TIMP-1 levels and tonsillar HPV positivity.

Conclusions:

  • Systemic MMP-8 elevation is linked to chronic tonsillitis.
  • Elevated S-MMP-8 and S-TIMP-1 may serve as potential biomarkers for tonsillar squamous cell carcinoma.
  • HPV status does not appear to influence MMP-8 or TIMP-1 levels in this context.
