In the UK, inhaled corticosteroids (ICS) are frequently prescribed for initial chronic obstructive pulmonary disease (COPD) maintenance therapy, contrary to guidelines. However, long-acting muscarinic antagonists (LAMAs) are increasingly used as a preferred first-line treatment.
Area of Science:
- Pulmonary Medicine
- Pharmacotherapy
- Real-world Evidence
Background:
- Clinical guidelines advocate long-acting bronchodilators for initial chronic obstructive pulmonary disease (COPD) maintenance therapy.
- Inhaled corticosteroids (ICS) are recommended for severe COPD or frequent exacerbations.
Purpose of the Study:
- To investigate real-world prescribing patterns of first maintenance therapy for COPD in the UK.
- To compare prescribing practices against current clinical guidelines.
Main Methods:
- Analysis of UK Optimum Patient Care Research Database data from 2009-2012.
- Inclusion of patients with a new COPD maintenance therapy prescription and prior COPD diagnosis.
- Characterization of patients based on demographics, disease history, symptoms, comorbidities, and treatment, stratified by disease severity and GOLD group (A-D).
Main Results:
- Long-acting muscarinic antagonists (LAMAs) were prescribed as monotherapy for 40.2% of patients.
- ICS, alone or in combination, were prescribed to over 40% of patients across all GOLD groups.
- ICS-containing regimens were given to patients with comorbidities and pneumonia history, where risks might exceed benefits.
Conclusions:
- Real-world prescribing shows ICS are often used outside guideline recommendations for newly diagnosed COPD patients in the UK.
- LAMAs are increasingly prescribed as first maintenance therapy, aligning better with guideline recommendations.
