Insights

Markedly elevated serum Carbohydrate antigen 19-9 (CA19-9) levels in two patients were linked to pulmonary sequestration. Surgical removal of the lung lesion normalized CA19-9 levels, suggesting sequestration as the cause.

Area of Science:

  • Thoracic Surgery
  • Medical Diagnostics
  • Oncology

Background:

  • Pulmonary sequestration is a rare congenital lung malformation.
  • Elevated serum tumor markers can sometimes be associated with benign conditions.

Observation:

  • Two young women presented with extremely elevated serum Carbohydrate antigen 19-9 (CA19-9).
  • Imaging revealed lung lesions and anomalous arteries, leading to a diagnosis of pulmonary sequestration.
  • Positron emission tomography/computed tomography (PET/CT) scans were used to rule out malignancy.

Findings:

  • Lobectomy confirmed the diagnosis of pulmonary sequestration in both patients.
  • Post-surgery, serum CA19-9 levels rapidly returned to normal.

Implications:

  • This case series suggests that significantly elevated serum CA19-9 may be associated with pulmonary sequestration.
  • Pulmonary sequestration should be considered in the differential diagnosis of unexplained hyper-CA19-9 levels.
  • Further research is warranted to understand the mechanism linking pulmonary sequestration and CA19-9 elevation.

Overview of Carbohydrate Metabolism

Carbohydrate metabolism is a fundamental biochemical process that ensures a constant supply of energy to living cells. The most important carbohydrate is glucose, which can be broken down via glycolysis to enter into the Krebs cycle and eventually lead to the production of ATP through oxidative phosphorylation.
Glucose transport into cells is facilitated by a family of transport proteins called GLUT (Glucose Transporters). GLUT4 is the primary glucose transporter for insulin-stimulated glucose...

Serum Laboratory Studies, Stool Test, Breath Test

Gastrointestinal (GI) diagnostic studies are pivotal in confirming, ruling out, diagnosing, or staging various diseases, including cancers. Following diagnosis, allocating time for discussions with the patient and providing informational resources is crucial. Diagnostic assessments of the GI tract often occur in outpatient settings like endoscopy suites or GI labs. Preparation for these tests may include dietary restrictions, fasting, liquid bowel preparations, laxatives, enemas, and the...

Radiological Investigation III: Pulmonary Angiogram and PET Scan

Radiological investigations are paramount in the diagnosis and management of various pulmonary diseases. Two essential investigations are the Pulmonary Angiogram and the Positron Emission Tomography (PET) Scan.
Pulmonary Angiogram
A Pulmonary Angiogram is an invasive procedure involving injecting a contrast medium through a catheter threaded into the pulmonary artery or the right side of the heart to visualize the pulmonary vasculature. Computed Tomography (CT) scans have mainly replaced this...
