Transhepatic embolization effectively controls bleeding from peristomal varices in liver disease patients. This minimally invasive procedure offers a safer alternative to surgery for intractable hemorrhage.

Background:

  • Peristomal varices can develop in patients with chronic liver disease and surgical stomas.
  • Hemorrhage from peristomal varices has a lower mortality rate (3-4%) compared to gastroesophageal varices (30-40%).

  • Four patients with recurrent, intractable bleeding from stomal varices underwent transhepatic mesenteric vein catheterization and embolization.
  • Three out of four patients experienced no further hemorrhage for at least 5 months post-procedure.

  • Transhepatic embolization is a successful treatment for intractable peristomal variceal bleeding.
